Student Travel Forms
Note to Students: Your program administrator will provide you with instructions regarding forms that you must complete before traveling abroad. Be sure to verify which forms are required before completing any of the documents listed below.

Health Clearance
Health Clearance Forms and Instructions for Undergraduate Students
Health Clearance is required for participation in Harvard-sponsored international travel, defined as activities operated and/or funded by Harvard, or that involve academic work qualifying for Harvard credit. The form should be kept on file by the appropriate administrator for six years following the student's travel.

PLEASE REMEMBER: You must submit a description of your Harvard Travel Plans along with the Health Clearance Form to UHS or your health care provider.
Only the Harvard Health Clearance form (linked above) can be sent to UHS via the Medical Records Office. If your program requires any other health forms, you may need to contact UHS or your primary care physician to schedule an appointment or obtain signatures.

Travel Waiver and Release Forms for Independent Travel
One of the following three forms should be used when a student is traveling abroad with Harvard funding or for Harvard credit but Harvard is not organizing or leading the trip (e.g. study abroad, research or internship funded by a Harvard grant). The form should be kept on file by the appropriate administrator for six years following the student's travel.
College students traveling to a country or region not rated "Elevated Risk"
College students traveling to an "Elevated Risk" country or region
Note: Harvard College students may not travel for Harvard credit or with Harvard funding to a country or region determined to be “High Risk.” Students and administrators should consult the Harvard College Policy on Undergraduate Travel Abroad and the Travel Risk Ratings list of High and Elevated Risk Countries and Regions.
Harvard Graduate and Professional School Students
Travel Waiver and Release Forms for Harvard-Organized Activities
One of the following five forms should be used when a student is traveling as part of a Harvard-organized activity (e.g. field trip, Harvard-organized internship program). The form should be kept on file by the appropriate administrator for six years following the student's travel.
International Travel:
Required part of a Harvard Course
Field trip led or organized by Harvard
Internship program or other program led or organized by Harvard
Domestic Travel:
Required part of Harvard Course
Field trip led or organized by Harvard
